https://form.jotform.com/SJBPAnimalShelter/adoption-application is the link to submit an adoption application. Our shelter is open by appointment only. You must submit the form and be pre-approved to adopt to schedule an appointment. All dogs/puppies are sterilized, vaccinated, 6 months and over are heartworm tested, examined and microchipped. The shelter also deworms biweekly and provides flea and heartworm medication monthly while the animals are in our care. .
