Meet Bean Cake ACCT-A-222505! Bean cake is a 7-year-old male who came in as a stray on 6/9. Medical Notes: Per an exam on 6/10: Noted to be dehydrated, pale, and having ambulatory issues on triage exam 6/9. Objective: -increased corneal opacity, cataract OD. Crusting ocular discharge -multiple missing and loose teeth, unable to perform full oral exam due to patient compliance -grade III/VI systolic murmur. No overt arrhythmia, pulses strong and synchronous, MM pale pink and tacky, CRT less than 2 sec, mildly delayed skin turgor -hunched posture when ambulating with slight outturned stance of LHL. Hissing, reactive when palpating LHL. Unable to perform full orthopedic exam. MCS 1.5/3 -mildly unkempt fur coat and shiny with no ectoparasites or skin lesions detected -Allowed most handling, limited minutes for exam Assessment: - Suspect LHL pain - r/o arthritis vs injury vs less likely fracture - Cataract OD - Severe periodontal disease - Thin MCS - Grade III/VI systolic heart murmur - ELEs - r/o metabolic (hyperthyroidism) vs inflammation vs infectious vs neoplasia vs other - Hyperthyroidism - CBC/Chem17/Lytes: HCT 29.3 (L), creat 0.6 (L), ALT 464 (H), ALP 133 (H) - T4: 16.9 (H) Per an exam on 6/9: "Check R eye, can't walk well Objective: -Unable to do complete exam, Pt became very grumpy. Gave a dose of gabapentina and tried a bit later, Pt was a little better but escalated quickly and didn't want to cause excessive stress to Pt. Pt appears to have lost weight - has saggy skin around limb/torso. Sniffed food but not didn't eat. Hydration percentage may not be accurate since all factors couldn't be evaluated. -Body condition score of 2 -OD has large amount fo dried crusting near medial canthus and down towards side of nose. Entire pupil appears to be white or have a thick white film over it. Pt not amenable to full examination. NSF OS -Ears have mild ceruminous d/c and dirt AU -Could not evaluate further than noticing that mms are pale(not white and may be missing some teeth -Has stiff gait, mild ataxia Assessment: Senior cat with pale mm's, weight loss, dehydration, diseased OD and ambulatory issues. Needs more comprehensive exam and diagnostics. (Attempted to get blood but Pt resisted, vein blew right away and had no other staff to help so it was aborted). Per staff 6/9: "Pt has a cold with d/c OD . OD is also cloudy , possibly blind . Finder also states she was barely walking." ACCT Philly is located at 111 West Hunting Park Ave in Philadelphia.
